My name is Roger Deleon
I am the owner and founder of Deleon Signature Services, LLC. After working in the Durable Medical Equipment and Home Health fields of Healthcare during the Pandemic, I wanted to expand my care and support for others. I understand that those who seek a Notary are sometimes in a vulnerable situation. I am willing to work with all those who are looking for someone to listen while also having integrity in what they do.

$10 - per notarized signature/document
I-9 Verifications

Travel fee
Will vary based on location of appointment and adjusted from the Beaverton area (0.76 per mile). Please contact me for an estimate prior to scheduling.
Hospital and Facility Signings
$60 - Providence St Vincents, Hillsboro Medical Center, Kaiser Westside Medical Center and all Westside rehab facilities.
$70 Providence Portland Medical Center, Milwaukie Hospital, Wilammette Falls Hospital, Legacy Hospitals, OHSU and other rehab facilities within Portland and on the East Side of the Metro Area.
